Rule 6-5(3):06:
After a license or certificate is issued or renewed, within five days of a new covered individual becoming involved with a child care facility, the child care facility licensee or certificate holder must submit the identifying information. A fingerprint card, waiver and fee, prepared either by the local law enforcement agency or an agency approved by local law enforcement, shall also be submitted unless an exception is granted under (4) below.

Findings:
CBS/MIS forms were not submitted within 5 days of hire for Hillary, Kristen, Chloe. and Talia. These screenings have now been completed. In addition, CBS/MIS forms are not on file for Meredith. This is a repeat finding from an announced inspection on 11/26/07. If this rule is not in compliance by 6/13/08 a Civil Money Penalty of $100.00 per person will be assessed.
