Rule 100-5(1):06:
The provider shall maintain a clean and sanitary environment.

Findings:
Toys are visibly dirty in the toddler and two year old room. There is a substance on the floor in the extra room (the room between the school age and preschool room.) This rule was previously found to be out of compliance in 5/17/07, 2/5/08, 3/20/08, and 4/22/08. This rule must be in compliance by May 12, 2008. If this item is not corrected by May 12, 2008, an additional civil money penalty of $50 will be assessed.
