Rule 100-17(7)(e)(i)-(iii):06:
When administering medication, the provider administering the medication shall: immediately record the following information: (i) the date, time, and dosage of the medication given; (ii) the signature or initials of the provider who administered the medication; and, (iii) any errors in administration or adverse reactions.

Findings:
An interview with Gavin Fothergill established that medication for a child had not been administered as directed for two consecutive days. If this item is not corrected by 04/27/2007, a civil money penalty of $100.00 will be assessed.
