Personnel records for each currently employed care giver and staff shall include documentation of orientation training completion.

Findings:
Review of records and interview with the director and owners reveals 4 of 12 caregiver files do not include documentation of orientation training completion. Orientation documentation forms are not found for S-1, S-2, S-4 and S-12. Technical Assistance was given for this rule at a complaint on 8/22/06.
